Doves senior care promotes client safety through communication

Home care is a 24-hour job and is constantly evolving around the changing needs of our clients.

Good communication is imperative, with family and clients at the assessment, and continues as long as clients remain on services to ensure updates and safety measures are in place to meet their changing care needs.

The individual client’s level of care, frequency of visits, and hours of service are documented during our initial meeting, or assessment. A main point of contact is established and method of communication (call, text, or email) is confirmed. We document the referral resource, and any additional health services the client is receiving.

Assistive devices used at home and for outings to prevent falls are recorded on the care plan. Additional medical equipment may be recommended, and fall risks inside the home and solutions are discussed.

The client or contact is given the office phone number, which is available 24/7. Client changes, concerns, and questions are directed to the office, whether from the client, family member, or a staff member. Our compassionate and experienced office staff quickly builds trusting relationships through communication with clients and family members.

Our goal is to keep clients out of the hospital. Caregiver continuity is priority when scheduling services for our clients. Changes in health, mobility, weight, eating or sleeping habits are easier to spot when the same caregivers visit regularly.

Healthy meal preparation, medication reminders, transportation to doctor appointments, light housekeeping, exercising, and use of assistive devices can be used to help prevent hospitalization or readmission.

We communicate directly with case managers and discharge planners regarding patient’s care needs when transitioning home.
